Employer Submits the Injury Report by EMAIL, MAIL or FAX
To Report by EMAIL...
The employer can complete and email the online INJURY REPORT form. This is a form fillable PDF document and requires Adobe Reader to access this form. Please use the TAB KEY or the mouse to navigate between boxes. Those sections highlighted in RED are required fields and must be completed in order to email the Injury Report.
PLEASE NOTE...YOU WILL NOT BE ABLE TO SAVE THE COMPLETED INJURY REPORT TO YOUR COMPUTER. YOU MUST PRINT AND SAVE THE PAPER COPY OF THE INJURY REPORT FOR YOUR RECORDS.
To email the Injury Report click on the "SUBMIT BY EMAIL" link on the bottom of the Injury Report. Your email browser will automatically open up with the necessary information populated. Please do not change the Subject Line. You can add additional comments regarding the injury in the body of the email.
If you are unable to email the Injury Report, close your email browser, then print and fax the Injury Report directly to the fax number above.
We also recommend that the employer saves the INJURY REPORT template to their desktop to have access to the Injury Report when working off line.
To Report by MAIL or FAX...
The employer will complete a paper copy of the Injury Report and then mail or fax the Injury Report to Laundry Owners Mutual-Attention: Claims Department. A paper copy of the Injury Report has been included with your policy. If you are in need of another paper copy of the Injury Report please call the Claims Department, or click here to print a copy now.
When To Report An Injury
An injury report needs to be immediately filed with Laundry Owners Mutual when an employee gives notice to their employer of a possible work related accident or illness. The injury report needs to be filed even if the employer questions the validity of the alleged injury.
If an employer questions whether an injury is work related, has additional information regarding the claim, or has questions concerning the nature of the injury the employer should immediately contact our claims department.
Remember, it is the employer's responsibility to secure the information from the injured worker after receiving notification that an injury has occurred.
